Self Management of Chronic Conditions (SMoCC) service
Self Management of Chronic Conditions (SMoCC) service delivers The Coach Program®, which is a free phone coaching service that helps people with:
- coronary heart disease
- pre-diabetes
- type 2 diabetes
- COPD (emphysema or chronic bronchitis).
The program helps you manage your condition and reduce the risk of future complications and avoidable hospital admissions.
Your COACH is a Registered Nurse who will discuss treatment specific to your condition. This can include your health targets, treatments, and ways to manage your risk factors, such as
- blood glucose levels
- cholesterol
- blood pressure
- diet.
Your nurse will contact you by phone every 4–6 weeks over approximately 6 months, at a time suitable to you.
At the end of each session your nurse will send you a letter recapping the call you have had and confirming the time for your next session. You can ask for copies of this letter to be sent to any other health workers involved in your care.
You are also welcome to contact your nurse in between sessions if you have any questions or would like further information.
To join the program, you can either ask your doctor or nurse for a referral, or call 13 HEALTH (13 43 25 84) and ask to leave a message for a nurse in the SMoCC service and we will call you back.
